Types of Breast Implants
Saline
Saline is the same salt water that makes up most of our body’s fluids. Because the implant is filled with saline, it does not have the same soft feel of a natural breast. However, in the event of a rupture, the saline implant will deflate quickly, so it’s easy to know the status of your breast implant after it has been inserted. Saline implants tend to be the least expensive type of breast implant.
Soft Silicone Gel
The soft gel implant is the most widely used breast implant in the United States. The texture of the soft gel implant most closely approximates natural breast tissue. The implant is circular and, in profile view, looks like a dome. This can give a very full appearance to the breast.
Cohesive Silicone Gel
The cohesive silicone implant is also called the “gummy bear” implant. This type of breast implant has more cross-linking between the silicone molecules, meaning that the molecules are more tightly bonded, allowing the breast implant to maintain its shape in a variety of body positions (sitting up, lying down). As a result, the cohesive gel implant tends to be stiffer.

Under the Muscle vs. Over the Muscle
As with the type of breast implant you receive, whether it is placed under or over your pectoral muscle will be determined by your preferences and your plastic surgeon's recommendations. There are advantages and disadvantages to both choices:
- Under the Muscle: Placing the implant under the muscle gives the greatest amount of coverage over the breast implant, masking its appearance in the long term. Another benefit is a decreased risk of capsular contracture (internal scarring around the implant). However, there is more postoperative discomfort involved in placing the implant under the muscle, and there is a small risk that when your muscle moves over the implant, the muscle will be visible through your breast tissue and skin.
- Over the Muscle: Placing the breast implant above the muscle uses only the breast tissue, skin, and fat to cover the surface of the implant. If your breasts are a full B to C cup to begin with, then there is little risk that the surface of the implant will be visible. However, if you have smaller breasts, or if the implant is large, then placing the implant above the muscle may make it more visible. This will deepen the cleavage significantly, and some people find it appears less natural. Additionally, future mammograms may be obscured by the implant if it is placed above the muscle. The benefit of placing the implant above the muscle is that postoperative pain is decreased, and there is little to no risk of the muscle movement becoming visible.
Fat Grafting

Fat transfer breast augmentation involves taking fat cells from other parts of the body during liposuction surgery and injecting them into the breasts. Fat transfer is a good choice for women who are interested in only a modest increase in breast size and have excess fat in other areas they wish to remove.

How Does Implant-Based
Breast Augmentation Surgery Work?
Breast augmentation should only be performed by a board-certified plastic surgeon. After anesthesia is administered, the surgeon will make an incision underneath the breast or around the nipple. The incision location will depend on the patient's body and desired results.
After the incision is made, the saline or silicone breast implant is inserted through the incision and placed either beneath or on top of the pectoral muscle. The type of implant, and the placement of the implant, will be determined by the patient's preferences and the surgeon's recommendations
Once the breast implant is placed, the incision is sewn shut. The whole process usually takes one to two hours.
The Recovery Timeline
Each patient's recovery experience will vary based on the type of procedure. In general, breast augmentation requires three to four weeks for a full recovery. Patients should be sure to drink plenty of fluids and get lots of rest.

Which implant type is safer?
All implant types we offer are FDA-approved, meaning they're safe. However, if you're particularly worried about safety, we may recommend the saline implant. If this implant ruptures, the salt water is safely absorbed by the body. If a silicone implant ruptures, it has to be removed quickly.
What is capsular contracture?
The body forms a capsule of fibrous scar tissue around anything it recognizes as foreign, including breast implants. This capsule helps keep the breast implants in place after augmentation. However, if this scar tissue becomes abnormally dense and causes pain, discomfort, or distortion of the breast, you may have capsular contracture.
